• I am currently under the care of a physician or other health care practitioner.
• I, in partnership with my own physician or licensed health care provider agree to take complete responsibility for your physical and emotional health.
• I understand that no physician-patient relationship is established through my participation in this workshop. I understand that this workshop is not psychotherapy.
• I understand that this Medical Educational Intuitive Certificate Program will NOT dispense medical advice or prescribe the use of any technique as a form of treatment for physical, emotional, or medical problems without the advice of a physician either directly or indirectly. • *You agree to indemnify, hold harmless and defend, the Released Parties from and against, any and all claims, liabilities, damages and expenses of every kind or nature whatsoever arising out of or attributed, directly or indirectly, to your failure to disclose truthful, accurate and complete information relevant to the services provided by Mona Lisa Schulz.
• *This Agreement contains the entire agreement between you and Mona Lisa Schulz and supersedes all prior agreements and understandings, oral or written, with respect to the subject matter hereof. If any term or condition of this Agreement shall be invalid or unenforceable to any extent or in any application, then the remainder of this Agreement, and such term or condition, except to such extent or in such application, shall not be affected thereby and each and every term and condition of this Agreement shall be valid and enforceable to the fullest extent and in the broadest application permitted by law.
• I understand if I cancel my attendance, my fee is non-refundable under any circumstances.
• I understand that Mona Lisa Schulz reserves the right to end a participant’s training at any time and will refund the client's payment in full.
• I understand that this Medical Educational Intuitive Certificate Program will not teach me how to give a medical diagnosis, medical treatment or medical advice; I understand that in this Medical Educational Intuitive Certificate Program I will NOT receive a medical diagnosis, medical treatment, or medical advice; therefore I understand that while I am in this class I will not use Medical Educational Intuitive Consultation to provide a medical diagnosis, provide prescription, treatment or psychotherapy.
